CNT Verification Policy Update
Effective February 2026, CCAHM will require a processing fee for verification requests related to CNT records more than three years old. This change applies to archived records and reflects the additional administrative handling required.
Verification requests for CNT records from the last three years will remain free of charge.
Use the links below to request one of the following:
- Verification of your full CNT completion to be sent directly to a board, agency, or other entity
- A name change on your CNT record or certificate
These forms are for full CNT credentials only.
Do not use these forms for Review Course verification. Review Course results are sent automatically.
Login is required to access the forms.
If you completed CNT with CCAHM but do not have login information, email support@ccahm.org with your full name (as it appeared when you took CNT), date of birth, and approximate year of completion. Emailing is preferred, as your email address must be verified to issue login credentials.
Before submitting, log in to confirm your profile information is current and accurate.
CNT Verification for NCBAHM (formerly NCCAOM):
Before requesting CNT course verification, please make sure you have submitted your NCBAHM certification application on the NCBAHM website. If your application is not on file with the NCBAHM, we will not be able to process your request.

Please submit your form only once. Multiple submissions may delay processing. Processing may take up to 10 business days.
