CNT Certificate or Verification Request

Please use the link below to make an online request for any of the following (after being notified that you have successfully completed the full CNT course):

  • Replacement certificate
  • Name change
  • Early certificate
  • Verification to be sent directly to board, agency, or other entity

Login is required to access the form. If you took CNT with us but do not have login information, please contact us by email at support@ccahm.org; please include your full name (at the time you would have taken CNT), date of birth, and approximate year of your CNT, so that we can connect you to the correct profile and issue you a password.  Emailing is preferable to calling because your email address must be accurate for us to send your password.

Before submitting your request, please log in and review the information in your profile to ensure it is current and free of errors. Multiple requests may be made through the same form.  Note that there is a separate fee for each type of request, except for name change. Verification requests are mailed via USPS* or sent electronically depending on the entity selected. Please allow up to 10 business days for your request to be processed and sent to the desired entity. 

*Due to COVID-19, the USPS has shared that "products and packages may require more time to be delivered due to limited transportation availability." As a result, delivery of verifications by a certain date cannot be guaranteed. To ensure delivery by a certain date, requests being sent to certain entities can be mailed overnight for an additional fee.
